No refunds will be given after the start of the course and a cancellation fee of 20% will apply for those who register then rescind.
Step 2: After payment of the registration fee please print and complete the application form* below.

Step 3: After completing the signed application form, fax to (321) 674-7050 with a cover sheet to the attention of Professional Development.
Include an e-mail address and a daytime telephone number. The application must be signed and completed entirely.
