Professional Development Course Application
If you are a program administrator and would like to offer Continuing Education Units (CEUs) as a part of your course offering, you must complete this form for approval by the Provost of the university. In order to insure the proper granting of CEUs, and to establish and maintain permanent institutional records for individuals receiving CEUs, this form must be completed and approved prior to the granting of CEUs.
